The Ultimate Sleeve Maker allows you to create custom CD and DVD sleeves using standard (8.5\" x 11\") paper.
Use the editor to format the sleeve, supply your own background image (jpg, bmp, gif), use as a CD/DVD sleeve or slip it into a case to use as a cover.
To help you, it even prints with folding guidelines.

With The Ultimate Sleeve Maker 2.1, you can make custom CD and DVD sleeves using nothing more than your standard 8 ½ by 11-inch printer paper. The software comes with an editor that allows you to format your sleeve a bit, with a few limitations. You can even upload your own background image using .jpg, .bmp, or .gif format.
Once you have designed your sleeve, simply print it. The Ultimate Sleeve Maker prints the folding instructions right on the design. Fold it, and slip your CD inside. You can also put the printed, folded sleeve into a case and use it simply as a cover. All you need once you download the software is a working printer and paper.
You will find a few limitations on this download, however. Some of the features available in the full version are disabled on the free version. For instance, you will not be able to create a back sleeve for your CD. You also cannot change some of the pre-set options. The program also does not allow you to add borders.
Overall, if you are looking for a product that will allow you to make your own sleeves for CDs, without special paper or printers, and using your own images, then this one will work well. While there are a few drawbacks, it allows you to make a custom cover for your CD/DVD creations, without purchasing an expensive software package.
